The proportion of production changes that cause degraded service and require remediation such as rollback, hotfix, or incident response.
What it measures
Change Failure Rate is the percentage of production changes that lead to user-impacting degradation and require corrective action. Teams should define which remediations count, such as rollback, forward fix, emergency patch, feature-flag disablement, or incident declaration.
The metric should be interpreted with deployment frequency and change size. A low rate achieved by releasing rarely or hiding failures is not healthy delivery performance.
