A useful item explains the intended outcome, enough context to discuss it, and the evidence or acceptance conditions that would show success. Detail should increase as the item approaches implementation.

Common mistakes

• Treating the backlog as an unbounded request inbox.
• Confusing ordering with a permanent commitment.
• Keeping stale items that no longer support the product strategy.
